Referrals

Are you a professional wanting to refer a family?

If you are a Health Visitor, Doctor or any other professional working with families and you think someone would benefit from our service, you can make a referral on their behalf, with their consent. Please phone us on 01327 353139 to discuss the family situation and if a referral is thought to be suitable we will record some basic details about the family and their needs. Once we expect to have a suitable volunteer becoming available, we will contact you check that the family still requires support. If so, we will send you a referral form to complete with the family and return to us. This represents a continuation of the home-visiting support that Home-Start Daventry and South Northants has been providing to local families across all Threshold Levels.

If we have been unable to find a suitable volunteer within 3 months, we will normally write to you to ask whether the family would still like us to look for a volunteer to support them.

Additionally, following changes in the way Children’s Centre Services are being delivered across the County, Home-Start Daventry and South Northants is contracted by Home-Start UK to deliver the Action for Children Peer Mentoring Service within the 2 districts. This is an element of the Children’s Centre Service.

Key points of the Action for Children Peer Mentoring Service are:

  • The service relates to referrals only made through Children’s Centres managed by Action for Children
  • Families accepting this service are at Level 2 & 3 of the recognised Threshold Levels
  • Peer mentoring support is time limited to 6 months for each intervention
  • The service is delivered by volunteers, supported by a fully trained co-ordinator
